Welcome to White Glove Community Care, a participating provider with the New York City Early Intervention Program (NYCEIP). Weâre honored to partner with you as your child grows, learns, and receives the support they need during this crucial stage of development.
At White Glove we are doing our best to ensure that your child receives the care and attention that they deserve and are entitled to receive. With you as their biggest advocate, we are certain that White Glove can deliver on this promise.
The first step is to properly provide you with a clear summary of your rights. Please see below:
Right to Due Process/Dispute resolution
If for any reason, you are not satisfied with any part of the process, including the results of the evaluations or the services authorized at the Individual Family Service Plan (IFSP) meeting, you have the right to use due process procedures to settle disagreements or complaints about your childâs EI services. The Right to Appropriate and Individualized Program
Early Intervention evaluations, services, and supports are provided at no cost to you. Services must be tailored to your childâs unique needs and your familyâs priorities.
The Right to Informed Consent
You have the right to receive clear explanations about any evaluation or service before it happens. No evaluation or service can begin without your written consent, and you may withdraw your consent at any time.
The Right to a Timely Evaluation
Your child is entitled to a comprehensive, multidisciplinary evaluation to determine eligibility. This evaluation must be completed within the New York State-mandated timeline of 30 days after the date of the referral.
The Right to Participate Fully in IFSP Development
If your child is eligible for the New York City Early Intervention Program (NYCEIP), you are an essential member of your childâs IFSP (Individualized Family Service Plan) team. Your goals, concerns, and insights guide the plan.
The Right to Choose Your Service Providers
You may request a specific provider or agency and may also request a change at any time should the needs of your child change.
The Right to Services Delivered in Natural Environments
Services should occur in settings that are most familiar and functional for your child â such as your home, daycare, or community program â unless an alternative location is appropriate.
The Right to Confidentiality
Your childâs information is protected under the Health Insurance Portability and Accountability Act (HIPAA). His or her records can only be shared with your service coordinator and other clinicians assigned to your childâs case. Your permission is required if your childâs records are to be shared with anyone else, including your childâs pediatrician or other family members.
The Right to Written Prior Notice
You must receive written notice before any change is proposed or made to your childâs evaluations, services, or IFSP.
